Sarnia Terminal Walkabout- Safe Work Permit (SWP) and Field Level Hazard Assessment (FLHA) were conducted.- Construction activity taking place in the terminal was discussed and observed.- An excavation area within the terminal was protected with fencing.- Emergency evacuation procedures were verified, including alarm system, muster points and windsocks.- The Sarnia Terminal is a 9-1-1 facility and this was indicated on SWP and FLHA- Entire terminal perimeter was fenced and access was continuously controlled, including with electronic monitoring.- Crash gates were verified.- Personnel on site wore required Personal Protective Equipment.- Areas where hearing protection was required were identified to employees and others present.- First Aid: station, kit, eyewash and Automated External Defibrillator were verified.- Walking areas in terminal yard were kept free of accumulations of ice and snow or other slipping or tripping hazards.- Fire protection equipment was strategically located, as well as two water reservoirs (water ponds).- Pipeline Maintenance Shop was clean and free of tripping hazards with fire protection equipment- Self Contained Breathing Apparatus was inspected and verified to be in good working order.- Facilities visited were clean and functional.

Mile Post 1783.8804- A site specific safety orientation was conducted by the Company's responsible person and included hazards and controls.- All persons on site wore required personal protective equipment.- Hearing protection in form of ear plugs was verified. - Fire extinguishers were properly located and inspected.- Muster points were clearly indicated and windsocks clearly visible.- Smoking was controlled and restricted to designated safe area with disposal.- An outdoor privy (portable toilet) was in place.- The excavation area was adequately stable and sloped and protected by barrier and warning signage.- Warmth to employees working in the area was provided by plastic cover and heat pumped in.- The generator on site was properly grounded.- Extensive rig matting was used and in good order.- Vehicle movement and back-in parking was coordinated with spotters in accordance with Company and its Contractor's procedure.

Mile Post 1871.9678- A site specific safety orientation was conducted by the Company's responsible person and included hazards and controls.- Powerline overhead-markers were in place.- All persons on site wore required personal protective equipment.- Hearing protection in form of ear plugs was verified. - Fire extinguishers were properly located and inspected.- Muster points were clearly indicated and windsocks clearly visible.- Extensive rig matting was used and kept in good order.- Smoking was controlled and restricted to designated safe area with disposal.- An outdoor privy (portable toilet) was in place.- The excavation area was adequately stable and sloped.- The excavation was protected by black-out fencing as well as 24/7 security watch.- Warmth to employees working in the area was provided by plastic cover and heat pumped in by a large heater.- Electrical lines were safeguarded by markers and signage.- Vehicle movement and parking was coordinated with spotters.- Secondary containment was in place for generators.- The Company representative provided a detailed explanation of non-destructive testing undertaken.
Observation 4 - Dig Site 3

Nebo Road Clearing and Excavation- A site specific safety orientation was conducted by the Company's responsible person and included hazards and controls.- A representative of another pipeline company was on site as the lines shared the same right of way.- All persons on site wore required personal protective equipment.- Hearing protection in form of ear plugs was verified. - Safe distance to excavating equipment was discussed and observed.- The log book on excavator was confirmed to be verified daily.- Probing for pipeline locating was observed to be performed safely.- Portable gas detection equipment was used.- Muster point was clearly indicated and windsock clearly visible.- Extensive rig matting was used and kept in good order with tripping hazard points marked with paint.- Smoking was controlled and restricted to designated safe area with disposal.- An outdoor privy (portable toilet) was in place.- Lighting plants staged for later use were provided with spill protection. - Vehicle movement and back-in parking was coordinated with spotters.
