Company must take both worn slings and slings with either no tags or unreadable tags out of service. The slings need to be destroyed and I.D. tags removed per Enbridge LP/MP Safety Manual, version 2.4, section 19.5, Inspection and Maintenance.

Two workers in a crew were not wearing hearing protection in a work area where administrative controls clearly identified hearing protection was a requirement.
Company must ensure workers wear PPE as required and identified by Administrative Control signage placed in specific work areas that require hearing protection per Enbridge LP/MP Safety Manual version 2.4, section 7.6 Hearing Conservation.

Storage of Oxygen cylinder and Acetylene cylinder not properly separated .
Company shall ensure that Oxygen cylinders are not stored in same area as Acetylene cylinders, and that oxygen/acetylene cylinders are 6.0m or 20 feet apart from each other, stored in an outside environment per Enbridge LP/MP Safety Manual, version 2.4, section 20.6 Compressed Gas Cylinder Storage (Compressed Gas Cylinder Storage).
Observation 10 - CNC#6: Eyewash station bottle not completely filled

Company shall inspect and monitor eyewash stations to ensure sufficient amount of eyewash bottles in place, expiry dates have not elapsed, and that bottles are full per Enbridge LP/MP Safety Manual, version 2.4, section 11.1 First Aid Equipment- Eyewash Stations
Observation 11 - CNC#10: - Hydrovac was not bonded to an earth ground

Enbridge Safety grounding standard pertaining to Hydro-vac trucks, was not being followed, as the Hydro-vac truck was not bonded & grounded to earth.
Company shall ensure that Hydro-vac truck is is properly grounded as per Enbridge LP/MP Safety Manual, version 2.4 section 4.13 Bonding & Grounding.
