Construction inspection 29.8 kilometers of NPS 42 of the High Pine Expansion Project (South Loop). To verify compliance to (CSA Z662, OPR, and CLC). Additional focus on contractor oversight activities, site specific hazard mitigation methods identified in the company Safety Construction Manual.

NPS 42 pipe in yard is untarped with no protector caps on ends of pipe. Checked material handling pipe procedure, found that pipe was shipped via boat from Japan to Vancouver BC port, then went on rail from Vancouver to Camrose, AB where pipe was coated. Then transported from Camrose to Chetwynd, BC, the pipe was tarped during all transporting, with tarp being removed at receiving locations then retarped. The pipe is retarped prior to transport pipe via truck to project site.Laydown supply management area was inspected and in good condition. Pipe was secured on flat deck trailers and tarped in yard. Processes were in place for obtaining supplies, control zones were in place to manage parking of worker vehicles, equipment, and buses.Offices "trailers" were clean and well lit, two washroom cars were placed strategically between the office trailers, which made worker access to easily.There was an open area in the laydown area that was set up for a helicopter to land, and while NEB was on site this area was used to facilitate a helicopter to land and take off again.

Observation 2 - Rock Face

Rock Face - KP6+200Site was very rocky and steep. Rock is being ripped after blasting took place the previous day. Drill holes are loaded and packed and then detonated at a time when field staff are at a minimum (blast time are at 7am and 6pm, this may vary due to fire ratings that day). Security is on site at night, and charges are required to be detonated within 24 hrs. after placement. Security all day Sunday, as that is the normal day of rest for all construction staff. Trucks bringing in pipe were observed to be tarped while being hauled on trailers, tarps were removed once pipe was unloaded off trailers.
Observation 3 - Remote Medical Facility

Remote Medical Vehicle, with Occupational First Aider Level 3 situated at KP 5+200Certifications of First Aider checked and validated. Mobile Emergency Unit was well equipped, all required faciities were in place, inside compartments and patient transportation facilities were orderly and clean.First Aider presented NEB inspectors with a Field Level Hazard Assessment and this assessment was satisfactory and all fields had been completed. First Aider was questioned on Journey Management process for medics as they are first in and last out at site, they have a documented process that involves texting management prior to leaving site and again when they arrive at final destination "home".
Observation 4 - Excavation and Rock Truck Equipment

KP 6+200Two Excavators and one Rock Truck was checked for their daily equipment checks, they were satisfactory and complete. Environmental checks were completed on equipment, no leaks were evident, and spill kits were on the equipment that was checked. Fire Extinguishers were checked and annual and monthly checks had been completed, this was evident on the tags.
Observation 5 - Drilling Equipment

KP 7.0 Hydro crossings were identified by goal posts (flagged caution lines to show safe limit height for crossing under the power lines). Wash facilities (porta-toilets) were at site, and washrooms were well equipped and clean.NEB inspectors approached Rock Drill operators, gaining their attention, and operations were shut down to enable inspections of equipment to take place. Field Level Hazard Assessment was presented for review and signatures. Assessment was complete for each drill and was found to be satisfactory. When NEB inspectors checked with the drill operators, one of the drill operators had not performed the daily equipment checklist, see CNC for equipment inspections Surerus Murphy, Site Specific Safety Plan (SSSP).Upon questioning the Foreman and Supervisor that were on site, they admitted that they had forgotten to request equipment checks to be completed on that particular drill.The other drill operators had completed the check satisfactory.

Observation 7 - CS2 Hill

Remote Medical facility was at CS 2 laydown area, that included a Helicopter Pad, helicopter, operator, and First Aider that are ready to be engaged in Emergency or site construction work activities. Goal posts to identify Electrical Powerline hazards were installed at pipeline crossings were observed guarding and indicating height limitations upon driving under the power lines. Policy on whole program was to use three methods of crossing under power lines (spotter, horn, and goal posts).There was good signage to warn the general public travelling on highway 97 that construction and pipeline crossing was occurring on the access road.Double wall tanks were being used and spill containment, drip trays and spill kits were available. Fire extinguisher was also located close to the refueling site.Coat was being conducted using induction heating. Ultrasonic testing was also being conducted as well as some grinding. Safe work was being conducted at all these sites. 42 " Pipe Welding activities for both manual "stick" and automated was observed.At 28+100 there were two explosive magazines located an appropriate distance of the access road and right-of- way. Security was discussed as being present at night and on Sunday which is a day of rest. These were licensed magazines and they were locked and signage was in place.There was a helicopter landing site at the staging area that could be used for transport to the top of CS2 Hill as well as for a possible medivac if required.At the CS2 hill there were stages of Hasco barriers (two sets) installed to prevent rock/equipment from rolling down on crews. Access to the hill was restricted and comprehensive FLHA was presented for review and sign-off. NEB staff did not access the hill as it was very steep and operations would completely shut down due to the hazards on the hill if we accessed it via a tracked vehicle. Steep slope specialists were contracted for this operation on the hill construction execution. At the base of the hill there was ample layout areas that separated crews and was very orderly and well-staged. Abundant rig matting was used. Controlled vehicle parking and access. I was a very organized operation with drilling, blasting and rock removal progressing. Pipe was being assembled at the base of the hill but ditch was still being constructed on the hill.On departure from the site NEB inspectors witnessed the arrival of a rock crusher that would allow rock to be crushed and used a bedding for the pipe. Communications were monitor and as the access was narrow, safe and clear communications and travel of the heavy equipment was witnessed.Operations at this site were very organized and layout size allowed for a control of heavy equipment and vehicles.
