Discussed role and responsibilities with Westcoast backfill inspector. Inspector was able to describe activities performed including safety related tasks such as ensuring project personnel did not enter trench to remove rocks in areas not prepared for entry, pipe end caps were installed where required, barricades and signage was installed around open trench, PPE was worn. It was noted that allow the inspector indicated that he does perform these tasks he does not generally note these actions on his inspection report. It was also noted that several site staff were not wearing hearing protection at the time of the NEB inspection in an area where it may have been appropriate.Interviewed Westcoast’s NDT company Lead Inspector. He provided an explanation of measures taken to identify and address hazards associated with NDT work activities. This team does complete their own daily Field Level Hazard Assessment (FLHA) as well as reviewing and signing off on the prime contractor’s FLHA for the work activities associated with NDT.Observed an “Ozzie Padder” in operation. It was noted that workers associated with this operation were not wearing hearing protection at the time of the inspection. A hazard assessment related to noise exposure was not available at the time of inspection.Observed a SB200 sideboom caterpillar being used to move a section of 42” pipe following completion of a field bend. The pipe was carried via the sideboom with one sling. The sideboom operator intended to carry the pipe up an incline but had difficulty maintaining the pipe in an alignment that ensured the leading edge of the pipe would not dig into the slope. A worker attempted to use his body weight (by stepping into the pipe end) to keep the trailing end of the pipe sufficiently low to the ground to avoid having the leading end make ground contact. This activity was in progress as the NEB inspectors arrived on site. The worker was observed with both feet in the pipe as it was suspended and moving forward, as well as with one foot in the pipe end and one leg trying to balance the load. The hill incline became too steep and the worker could no longer use his body as a counterweight to keep the pipe aligned.The pipe was laid down and a discussion commenced amongst Westcoast project staff, contractor staff and NEB Inspectors to determine a safer means of moving the pipe. There were no tag lines in use, and the terrain conditions were slippery. The worker stated that the tag line was wet and muddy and he could not maintain a grip while using it. It was determined that a second side boom would be utilized to complete the task safely. NEB inspectors met with Westcoast project safety personnel to review their responsibilities related to the High Pine Expansion Project, project incidents and activities, interaction with Westcoast inspection team, project management, and contractor personnel.
