A pre-work inspection of the crane/barge was conducted while the vessel was moored remotely from Westridge.General overview of main deck conducted. Workers were conducting preparatory work such as prepping marker buoys, painting and general housekeeping. The engines were silent and no lines were under tension which created a safe workplace to conduct the inspection and allowed access to all areas and enabled unobstructed access to high hazard work areas. No deficiencies noted.All workers in appropriate PPE which included personal floatation devices.Confined spaces were clearly marked.Crane operators log book up to date.Annual crane inspection up to date. Crane inspection plate was affixed but the documentation was not yet on site. The crane was inspected 14 Sept 2017 with the certification documenation signed 28 Sept 2017. This was not considered a non-compliance as the crane was not yet working.Rigging appeared to be adequate. Company policy is that all rigging is inspection and tagged prior to use.Work areas fenced. Defribulator case was empty. Defribulator was enroute to site. Sign was attached to the case to notify workers of the location of secondary defribrulator.First aid kits available. Fire extinguishers with up to date inspections.

Observation 2 - I-beam trolleys

I-beam trolley's in engine room had not been inspected nor were they weight rated. They were not in use and the inspection officer was informed that they were "never used". The company locked and tagged out both trolleys.
