Observation 7 - Information Request #2 - Pipe Supports
NEB inspectors noted, in relation to valve site 13F-401 W, that the some pipe supports directly under flanges are isolated by a rubber-like sheet in between the ¼ circumference support and the flange. There were signs of water and moisture insertion and “trapping” between the crevice of the flanges and the supports. NEB Staff are of the view that this could create a potential hazard of crevice corrosion within the flanges, and require further information.
Due date: 2017-05-26
Observation 8 - Information Request #3 - Locked exit gate at Valve Site 10-F-101V
Discipline: Safety Management
NEB Staff observed that one of the two exit gates of the valve site was locked, and could not be readily opened from the inside without a key. NEB Staff is of the view that this may trigger a safety issue in case of emergency situation within the fenced area.
Reason closed: Full relief granted
