Observations:

• At time of inspection, there was one contractor company active on site (11 total staff), erecting a new storage tank.  The contractor activities had full-time oversight from their site foreman and PMC representatives were also present at location and provide direct oversight dependent on task.
• Job Hazard Assessments and Field Level Hazard Assessments were observed to be completed daily and posted.
• The contractor records were also reviewed.  The contractor is completing weekly general site safety inspections as well as daily focused inspections (e.g. general mobilization, rigging, weldiong, housekeeping).
• Copies of critical lift plans were reviewed and noted as complete for task.
• Inspection certifications for cranes were provided for review and found to be in order.
• A sampling of training records for employees (including those tasked with operating cranes) were reviewed and noted as current.
• Hazard Identification and Near Miss reports were observed to be posted on bulletin board.  It was noted that 4 previous near misses were related to ladders.  A site standdown and discussion regarding ladder safety had occurred.
• Fire extinguishers and first aid kits were noted to be adequately placed and maintained where observed.
• Recently installed piping was noted to not yet include any identifying marking as additional coating/surface work was to be completed.  pre-existing piping was noted to be adequately marked where observed.
